In November 2025, MDDUS brought together doctors, educators, regulators and support organisations for a dedicated IMG Conference focused on the lived experiences of International Medical Graduates (IMGs) working across Scotland’s NHS. As hosts, we set out to create a space for honest discussion about the challenges IMGs face, from adapting to new clinical environments to navigating cultural, organisational and regulatory expectations. The event made one thing clear: supporting IMGs effectively is essential to a fair, safe and thriving health system.
Insight from Dr Naeem Nazem, Head of Medical at MDDUS
Drawing on his long-standing work supporting IMGs across the NHS, Dr Nazem reflected on the themes that emerged throughout the event:
“I have worked alongside International Medical Graduates throughout my career. They arrive here with skill, resilience and ambition, yet face the challenge of adapting to a new country, a new culture and a new health system. And all while trying to prove themselves from day one.”
Dr Nazem noted that many IMGs continue to experience unsympathetic attitudes or bias. Recent MDDUS insight shows that 58% have experienced racist microaggressions, with many choosing not to report them. These pressures are real, shaping confidence, wellbeing and progression and reinforcing why a consistent, compassionate approach is needed.
Across discussions, several clear themes emerged:

Everyday challenges outside work
such as housing, transport and induction—can have a significant impact during the transition to life in the UK.
Systemic issues, not ability
drive many of the attainment and cultural barriers IMGs face.

Trust, fairness and consistent guidance
remain critical concerns, particularly around raising issues and navigating early career uncertainty.
Supervisors need clearer tools
time and support to guide IMGs effectively.
What needs to happen next

A national, standardised approach to IMG induction with earlier access to help, before issues escalate

Processes
