Care Navigation - transforming your practice

Taking place on Wednesday, 16 October, between 9.30am and 12.30pm, this interactive course is designed to support the whole team to successfully develop Care Navigation (signposting) in your practice.

This practical workshop is designed to support the whole team (front of house staff, as well as clinicians) to successfully develop Care Navigation (signposting) in the practice.

Via a mix of presentation, case studies and small group discussion, the following will be explored:

  • What Care Navigation is…and, importantly, what it is not
  • Creating partnerships and opportunities for person-centred conversations
  • Building on the fact the GP is not always the first person a patient needs to see – creating patient choice to be seen by the right person, at the right place and at the right time
  • Recognising red flags; maintaining confidentiality and protecting patient safety
  • Understanding and managing patient expectations - reframing how patients think about contacting the practice, and the role of front of house staff
  • The importance of developing a consistent, ‘whole team’, approach to Care Navigation
  • Developing positive communication skills for Care Navigation
  • Identifying methods for explaining and communicating Care Navigation to your patients
  • When the patient calls the practice... The impact of a GP recorded message; what to say when answering the phone
  • Mapping your local services; creating a directory of services to care navigate to
  • Collecting information to know how well you are doing – and celebrating success!

At the end of this workshop, delegates will feel better equipped and more confident to move forward with Care Navigation in the practice.

Upon completion, a certificate detailing 3 hours of verifiable CPD will be provided by MDDUS to all attendees.

To take part in this workshop:

  • You must be able to access and join a Zoom meeting from whatever device you choose to use.
  • Participants require a PC or laptop which has either a webcam and microphone integrated (or attached). Alternatively delegates can use a tablet/phone (although some functionality may be unavailable and you may not be able to see all delegates).
  • Participants must be willing to share their names and webcam with other participants whilst attending the workshop.
